For many children on Medicaid, hospital wait times can stretch 6 to 18 months, turning manageable conditions into prolonged pain and delayed development. SmileMD was founded in 2014 by three anesthesiologists who saw this inequity and set out to change it by changing the site of care.

We partner with community dental practices to bring pediatric anesthesia into their offices, shortening Medicaid wait times from months to weeks while holding the clinical standards families expect for a surgical procedure.

Inside Our Clinical Model

The working unit of our model is the Smile Day: a full day of scheduled cases at a partner dental office, run by a three-person clinical team. A centralized operations team handles everything around it, from pre-admission testing to post-procedure follow-up. More than a change of setting, this model changes the experience for everyone involved.

A child’s safety on a Smile Day does not rest on one person or one piece of equipment. It’s supported by a proven, AAAHC-accredited clinical system that runs the same way in every office.

An Experienced Medical Team on Every Case

Every case is led by an anesthesiologist, with a pediatric-focused recovery nurse and a paramedic beside them. Before the first child is brought back, the anesthesiologist and treating dentist align on the schedule, patients, and care
plan.

Certified Equipment
& Medication Tracking

Our anesthesia machines are biomed-certified and maintained by a dedicated logistics team. Every medication is tracked by lot number across our network, so a recall can be traced to every case it touched. Nothing arrives on a Smile Day unchecked, and nothing arrives without a backup.

3-Stage Patient
Clearing Process

Our patient care coordination team collects and screens each patient’s health history, requesting a physical when indicated. Anything that flags goes to a nurse for review. Final say rests with the treating anesthesiologist running the case.

AAAHC-Accredited Standards

SmileMD holds accreditation from the Accreditation Association for Ambulatory Health Care, an independent evaluation of the organization’s clinical and operational standards. Accreditation is voluntary, reviewed on an ongoing basis, and verified by an outside body.

Follow-Up After Every Case

A nurse remains with each child through recovery and reviews discharge instructions with the family before departure. The following day, a nurse calls to assess recovery and answer questions, giving families a direct line to a clinician and routing anything that needs attention back to the dental office.

Continuous Improvement

Clinical education at SmileMD is ongoing. Our providers train on current anesthesia practice throughout the year, protocols are reviewed and updated as standards of care evolve, and lessons from cases across the network feed back into how every team operates.
